Data about fall frequency and circumstances were collected by retrospective recall and fear of falling was assessed by the modified Falls Efficacy Scale.

Results:
Compared with healthy subjects, EDS-HT subjects showed significantly impaired balance, reflected by increased sway velocity, medio-lateral and anterior-posterior sway excursion, and sway area during mCTSIB and TS. Gait velocity, step length and stride length were significantly smaller during all walking conditions and a significant dual-task-related decrement was found for gait velocity, step and stride length, and cadence in the EDS-HT subjects compared to the control group. Ninety-five percent of the patients fell during the past year and some fear of falling was measured.

Conclusion:
This study is the first to establish that EDS-HT is associated with balance and gait impairments, increased fall frequency and poorer balance confidence, implying a decrease in the safety of standing in everyday life situations. Whether these deficits can be improved by appropriate exercise programs needs to be addressed in future research.

Objective: The criteria for Ehlers–Danlos syndrome (EDS) and the hypermobility syndrome (HMS) should be reliable. Examination for general joint hypermobility has high reliability but there is only sparse information on the reliability of skin tests, and no information on the level of normal skin extensibility. The present study aimed to assess skin signs by means of clinical and para-clinical methods.

Methods: A total of 31 EDS patients and 28 healthy controls were examined blinded and in random order. Inter-examiner analysis of clinical tests for skin extensibility, consistency, scarring, and bruising was performed, followed by analyses of extensibility with the suction cup (SC), consistency with a soft tissue stiffness meter (STSM), and thickness with ultrasonography (US). Semi-quantitative assessment of skin extensibility in healthy controls was incorporated in the tests.

Results: The clinical analyses demonstrated kappa values of: 0.72 for extensibility, 0.23 for consistency, 0.53 for scarring, and 0.63 for bruising. Skin extensibility measurements in healthy controls (n = 28) were 2.79 and 2.93 cm (mean + 2 SD), respectively, by the two examiners. There were significant differences between patients with classical-type EDS and controls with respect to skin extensibility by SC (4.91 vs. 12.52 kPa/mm) and skin consistency by STSM (0.59 vs. 0.76 N). We found no difference in skin thickness.

Conclusion: The reproducibility of the clinical skin tests was substantial to good, apart from the consistency measurements. We suggest that skin consistency is withdrawn as a diagnostic criterion. The upper level for normal skin extensibility should be 3 cm. SC and STSM are promising para-clinical methods, but their diagnostic sensitivity and specificity need to be determined.

Belangrijkste bevinding: deze onderzoekers vinden dat vanaf 3 cm sprake is van abnormale rekbaarheid van de huid.

Ik vind dat deze vrouw erg irritant praat maar haar onderzoek is zeker interessant: http://prettyill.com/videos/watch/why_eds_pots_patients_are_so_ill_relief_the_driscoll_theory

Het is een theorie over POTS in EDS patienten. Deze arts denkt dat het te maken heeft met een lage overdruk van de hersenvloeistof die mogelijk ook de vaak voorkomende trage ontwikkeling van kinderen met EDS veroorzaakt.

Ik kwam dit artikel tegen in een facebook groep, ik vind dit het beste wetenschappelijke artikel over EDS type 3 dat ik ooit gelezen heb, het enige wat ik echt mis zijn de geassocieerde blaasproblemen.

Dit ga ik zeker weten aan mijn huisarts geven, misschien dat het voor wat meer begrip kan zorgen...

Ziet er als een heel duidelijk document uit. Ga het ook uitdraaien. Dat van die blaasproblemen mis ik ook. Maar misschien is het wel niet iets wat heel specifiek bij een van de types van EDS hoort. Maar als je iets vind houd ik me echt aanbevolen. Kan ik wel gebruiken bij mijn uroloog. Niet dat hij het slecht doet, maar het is altijd fijn als je iets duidelijks op papier hebt.

In het UZ Gent heb ik al aan verschillende onderzoeken deelgenomen , ook aan dit onderzoek, ik vind het heel goed dat er zoveel onderzoek naar gedaan wordt, momenteel ben ik weer bezig met deel te nemen aan een studie, ik hoop dat op die manier er ooit toch iets gevonden wordt om een echt behandelplan op te stellen.
De onderzoeken zijn soms best pittig en ook wel confronterend, maar het is ook fijn om dan met andere mensen te kunnen communiceren die hetzelfde hebben.
Wordt dit soort onderzoeken ook in Nederland gedaan?

Ik kreeg deze toegestuurd: Magnesium and EDS. Het is geen wetenschappelijk onderzoek maar er staan wel een hoop onderzoeken in gelinkt en het is door een arts geschreven.

Samengevat staat er dat magnesium tekort onder de algemene populatie veel voorkomt en het bij eds een extra aandachts punt moet zijn omdat het klachten die met eds geassocieerd worden behoorlijk kan versterken. Problematisch is echter dat magnesiumwaarden slecht te testen zijn omdat de bloedwaarde eigenlijk niet representatief is voor de voorraad magnesium in je lichaam. Het is giswerk en proberen dus.

Nog een interessant artikel gevonden: autonome sysfunctie in patienten met het hypermobiele type van EDS
Om even een samenvatting te geven:
Autonome dysfunctie blijkt veel voor te komen onder de relatief kleine testgroep patienten met eds-hypermobiel. Dit is niet onverwacht omdat gelijke bevindingen zijn gedaan bij aandoeningen met vergelijkbaar klachtenpatroon (hms, fybromialgie, cvs).

De oorzaak is niet geheel duidelijk, er kunnen verschillenden zijn.

O.a. medicatie die veel voorgeschreven wordt bij eds (opiaten zoals tramadol, anticholinergica zoals oxybutinine en tricyclische antidepressiva zoals mirthazopine) heeft invloed op de werking van het autonome zenuwstelsel en kan autonome dysfunctie tot gevolg hebben.

Ook kunnen vermiderde fysieke activiteit, depressie en angststoornissen die bij eds meer dan gemiddeld voorkomen problemen geven met het autonome zenuwstelsel.

Een andere mogelijke oorzaak zijn zenuwbeschadigingen door luxaties, zwak bindweefsel rond de zenuwbanen en hypermobiele wervelkolom, waar onder andere ook de veelvoorkomende zenuwpijn aan toegeschreven wordt.

Het is belangrijk om autonome dysfunctie te diagnosticeren en te behandelen omdat de hartritmestoornissen (hartkloppingen, pots achtige verschijnselen) een risico kunnen vormen bij operaties en autonome dysfunctie de algemene kwaliteit van leven behoorlijk kan beinvloeden.

In het tweede artikel wat jij aangeeft worden oorzaken gegeven voor de dysfunctie van het autonome zenuwstelsel. Deze oorzaken kunnnen allemaal van invloed zijn. Maar wat ik een beetje mis is dat het autonome zenuwstelsel bij EDS ook al eerder is ontregeld omdat de steunweefsels om de bloedvaten en zenuwen slechter zijn. (Kwa bloedvaten heeft dat veel invloed op de bloeddruk bij het opstaan) POTS (een vorm van autonome dysfunctie) komt dan ook vaak voor bij EDS. Dat ontstaat niet door de oorzaken die genoemd worden in jouw samenvatting van het artikel. Die oorzaken kunnen de POTS klachten verergeren, maar de POTS zelf hoort gewoon bij de persoon en hangt samen met de EDS. Het is een onderdeel ervan.

Mijn samenvatting was ook even snel uit het hoofd ;) Bedankt voor de aanvulling.

Ze halen een aantal onderzoeken van dr. V (neuroloog in de ved adviesraad) aan om de mogelijke oorzaken vanuit het slechte bindweefsel te omschrijven.

Maar het onderzoek is volgens mij voornamelijk gericht op het voorkomen van autonome dysfunctie bij eds-III, niet de oorzaak ervan.Ze geven wel een hoop mogelijke oorzaken, dat wel.

Oke dat klinkt wel interessant. Ben benieuwd naar hoe zij het willen voorkomen, zal het artikel dan eens volledig lezen zelf. Want ik heb daar toch best last van.

Ze gaan helaas niet duidelijk in op behandelmogelijkheden, maar van de manier waar ze er over praten heb ik wel het idee dat er iets aan te doen is. Ik ga er zelf nog eens op zoeken, behandeling van autonome dysfunctie. Wie weet...
